Preparing for El Niño Climate and Health Responder Course: Disaster Risk Reduction and Adaptation in the Americas and the Caribbean

Introduction
The Americas and the Caribbean are the second most exposed region in the world to extreme weather and climate events, with 74% of countries classified as highly exposed to climate-related hazards. The convergence of worsening climate change, a strong El Niño, and increasing exposure and vulnerability to simultaneous and cascading hazards is expected to heighten the risk of health impacts and disasters across the region. Recent years have brought record-breaking heat, severe droughts, catastrophic floods, hurricanes, devastating wildfires, and outbreaks of climate-sensitive diseases, resulting in significant loss of life, displacement, damage to health systems, and disruption of livelihoods. The region experiences more than 21,000 heat-related deaths annually, while climate extremes have contributed to record dengue transmission, food insecurity, and increasing disaster-related displacement.
This regional course is designed to strengthen the capacities of health professionals across the Americas and the Caribbean to understand, assess, and manage disaster and climate-related health risks through integrated disaster risk reduction (DRR) and climate change adaptation approaches. Grounded in frameworks and guidance from the United Nations Office for Disaster Risk Reduction, the World Health Organization, the Pan American Health Organization, the Sendai Framework for Disaster Risk Reduction, and others, the course’s 14 sessions focus on preparing the health and correlated sectors for the most relevant climate hazards affecting the region, including extreme heat, floods, hurricanes, droughts, wildfires, vector-borne diseases, air pollution, and climate-driven displacement with coordinated cross-sector strategies, tools, and frameworks for strengthening preparedness and reducing risk.

Course Organizers
The Climate and Health Responder Course: Disaster Risk Reduction and Adaptation in the Americas was developed through a collaboration among the Global Consortium on Climate and Health Education (GCCHE), Pan American Health Organization (PAHO), United Nations Office for Disaster Risk Reduction – Regional Office for the Americas and the Caribbean (UNDRR), Inter-American Institute for Global Change Research (IAI), Ministry of Health of Brazil (Ministério da Saúde), Health Canada (HC/SC), Latin American and Caribbean Hub for Climate Change and Health (CLIMAS), International Federation of Red Cross and Red Crescent Societies (IFRC), Pacific International Center for Disaster Risk Reduction, and EarthMedic and EarthNurse Foundation for Planetary Health. Through the collective expertise of this partnership, we have designed a course that can provide foundational knowledge and actionable strategies that disaster professionals and health practitioners can immediately integrate into daily practice and planning.
Audience
The training is tailored to professionals from ministries of health, public health agencies, national institutes of health, emergency preparedness institutions, disaster risk reduction and management authorities, local governments, hospitals, community health organizations, academia, and civil society in the Region of the Americas and the Caribbean. All health professionals are welcome to join.
Course Primary Language: Sessions will be primarily offered in English with live interpretation in Spanish, and Portuguese, and closed captioning in French.
Certificate of Participation Requirements
Participants who attend at least 9 of the live sessions and pass the final exam with a score of >70% will be awarded a Certificate of Participation in climate and health from the course partners.
The exam link will open on December 10, 2026 (the date of Session 14, the final session) and will close seven days later, on December 13, 2026. Participants should be prepared to take the exam in one sitting. Certificates will be awarded by January 10, 2027. The exam will be offered in all course languages and will consist of Muliple choice and true/false questions.
Course Structure
The course consists of 14 sessions combining lecture-based instruction with case studies drawn from across the Americas and the Caribbean. Resources such as frameworks and suggested readings are provided to all course participants.
Session 1 – October 13: Foundations of the Climate-Health Nexus on Disaster Risk Reduction in the Americas and the Caribbean
This session introduces the climate-health nexus and the core concepts and frameworks that underpin climate and health risk reduction in the Americas and the Caribbean, including the principles of the Sendai Framework for Disaster Risk Reduction and their application to climate and health hazards. Participants will explore how hazards, exposure, vulnerability, resilience, adaptive capacity, and risk interact to shape climate-related health impacts, and how social, economic, environmental, and governance conditions influence unequal exposure and vulnerability across populations and territories. The session examines major climate-sensitive health risks affecting the region, including the role of poverty, inequities, unplanned urbanization, environmental degradation, and governance challenges in amplifying disaster and health risks.

Session 2 – October 15: Applying Disaster Risk Management to Climate and Health
This session introduces the disaster risk management approach and its application to climate-related health risks. Participants will examine how risk assessment, risk management, risk reduction, preparedness and readiness, early and anticipatory action, response, recovery, and post-event evaluation work together to reduce health risks before, during, and after climate-related emergencies. The session also explores cross-cutting capacities that support implementation across all phases, including monitoring and surveillance, early warning systems, emergency coordination, workforce readiness, and multisectoral collaboration, as well as climate and health risk communication and behavioral responses that encourage protective action.

Session 3 – October 22: Extreme Heat, Cities, and Population Health Protection
This session examines the growing impacts of extreme heat in urban environments and explores how cities can strengthen preparedness, reduce exposure, and protect vulnerable communities through risk-informed planning, early warning systems, and effective public communication strategies. Through an assessment of trends in heatwaves and mortality and morbidity rates, the session explores how heat impacts health, particularly for vulnerable populations, and how it creates additional hazards that disrupt access to essential infrastructure and basic services. The session also examines urban adaptation interventions and heat-health governance and policy implementation.

This session focuses on strengthening the resilience of health systems, health facilities, and the health workforce to extreme heat events. Participants will explore how increasing temperatures and more frequent heatwaves can disrupt health service delivery, increase patient demand, affect occupational safety, and strain energy and water systems. The session introduces practical approaches for strengthening heat preparedness and operational continuity across healthcare settings, including heat-health surveillance and forecasting tools, facility-level adaptation measures, and equity-centered preparedness approaches.

Session 5 – October 27: Wildfire Risk Reduction, Air Quality Preparedness, and Community Protection
This session examines how climate change, land-use change, and environmental degradation are increasing wildfire risk and smoke exposure across the Americas and the Caribbean. Participants will explore the health impacts of wildfire smoke and poor air quality, particularly among children, older persons, people with disabilities and chronic diseases, and outdoor workers. The session focuses on city-level adaptation and risk reduction measures, including wildfire risk mapping, fuel management, air quality monitoring, early warning systems, clean air shelters, and community preparedness planning.

Session 6 – October 29: Health Facility Preparedness and Workforce Response to Wildfire Smoke and Air Pollution
This session focuses on strengthening health system resilience to wildfire smoke and air pollution events. Participants will examine how smoke events affect healthcare demand, facility operations, workforce safety, and continuity of essential services, including preparedness measures such as indoor air quality management, filtration systems, smoke preparedness plans, surge capacity planning, and occupational protection. Special attention is given to protecting healthcare facilities, schools, long-term care facilities, and other critical services during prolonged smoke events.

This session examines the growing health risks of floods and other hydro-meteorological hazards across the Americas and the Caribbean, particularly in urban and coastal settings and among populations experiencing greater social vulnerability. Participants will explore how climate change, urbanization, environmental degradation, land-use patterns, and social inequities shape flood exposure and vulnerability. The session introduces flood preparedness and early warning approaches, city-level risk reduction and adaptation measures, and examines how risk communication, trust, and misinformation influence responses to warnings and evacuation messages.

Participants will examine how floods can disrupt healthcare delivery through damage to infrastructure, loss of electricity and water services, supply chain interruptions, and contamination risks. The session explores strategies for climate-resilient health facility planning, workforce preparedness for managing flood-related injuries and diseases, and the importance of surveillance and integrated preparedness planning.

Participants will explore how climate change, changing precipitation patterns, environmental degradation, and increasing pressure on water and food systems are intensifying drought risks and affecting health, livelihoods, nutrition, mental health, and community resilience, as well as preparedness and adaptation strategies that strengthen health system resilience to prolonged drought conditions.

Climate-related disasters rarely occur in isolation. A single event can trigger cascading impacts across health, water and sanitation, energy, transportation, supply chains, food systems, and healthcare services, while also increasing secondary risks such as infectious diseases, landslides, displacement, and disruptions to essential care. This session examines how cascading and compounding risks develop and how failures in one system can generate consequences across others, including how post-disaster recovery and Build Back Better approaches can reduce future risks rather than restore previous vulnerabilities.

Participants will explore how heat, changing rainfall, floods, droughts, environmental change, and population displacement can increase risks of diseases such as dengue, malaria, Zika, chikungunya, leptospirosis, cholera, rabies, and tetanus, and how climate-informed surveillance, early warning, One Health approaches, and health-system preparedness can strengthen prevention and response.

Participants will examine how climate-related health risks require coordinated action across health systems, disaster management agencies, urban planning sectors, finance ministries, academia, civil society, and affected communities. Multilateral, national, and subnational case studies illustrate how these components can be integrated within health plans—from risk reduction and adaptation to preparedness, emergency response, recovery, and post-event learning.
