There are several crucial and time-sensitive steps that need to be completed in order for us to enroll you into the program. If you haven't already, please complete the CSRI 2025 Admitted Students Questionnaire as soon as possible (note that this is separate from the MSPH Decision Reply Form in #1 below).

Submit your MSPH Decision Reply Form
(May 23 deadline)By May 16, you should receive an email from the Mailman School of Public Health (MSPH) which will let you know that your application status has changed and will provide a link with log on credentials for you to view your updated application status. Once you log on, you will be able to view your decision letter. The CSRI Decision Reply Form* must be completed and returned ASAP so that the school can process your official enrollment as a non-degree student. This is a different from CSRI's Admitted Students Questionnaire.* If you have been admitted to a degree program, please complete the Decision Reply Form for the degree program. -
Student Health Pre-registration Requirements
(June 10 deadline)-
As soon as you have been notified of your acceptance to the CSRI program, have your provider (an MD/DO, NP, or PA who is not a relative or yourself) complete and sign the Pre-Registration Immunization Form for Non-Clinical Students.** The June 10 deadline is critical for the Pre-Registration Immunization Form. Please have your provider complete the form even if you have not been able to complete the MSPH Decision Reply Form.

In early June, you should have access to the Student Health Portal where you will upload your immunization form and proof of COVID-19 vaccination. Note that you should select CUIMC Student even if you are also a CUIMC employee. Even if you do not have access to the Student Health Portal, please make sure your immunization form is completed and ready to upload by the June 10 deadline.

For CUIMC/NYPH employees only:  Health & Related Service Fee Waiver
(June 10 deadline)-
Once you have been successfully enrolled at MSPH (requires a few days after completing the official MSPH Decision Reply Form), complete the Health & Related Service Fee (HRSF) Waiver and submit to shsinsurance@cumc.columbia.edu.

Please also send a copy to csri@columbia.edu so that we may follow up with Student Health on your behalf in case you are charged the fee.
The Health and Related Services Fee (HRSF) is a new University fee mandated by the Board of Trustees. The HRSF, which is $418 for the summer, grants access to all services and resources provided by Student Health on Haven. However, if you are a CUIMC and/or NYPH employee, you already have access to Workforce Health and Safety and may choose to waive this fee. This fee is not included in CSRI’s tuition and fees and if you choose not to submit the waiver, you will be responsible for payment of the fee.

OTHER THINGS TO KNOW:
- You will be asked to provide initial plans for a project you plan to pursue for the Grant Writing course. The CSRI Initial Project Information assignment is due on June 22. You are encouraged to discuss potential projects with your mentors before completing this pre-CSRI assignment.
- You are required to keep 8 am - 6 pm on weekdays dedicated to CSRI in order to attend lectures, labs, one-on-one mentoring sessions, and office hours. Additional time will also be required to complete assignments. We also highly recommend that at least one day on the weekend be blocked off for dedicated time to work on your grant writing project and prepare for discussions in the upcoming week.
- As part of the CSRI program, you will be registered for the following courses in mid-June (pre-registration requirements detailed above must be completed):
- P6104 - Introduction to Biostatistical Methods
- P6400 - Principles of Epidemiology
- P8221 - Decision Analysis and Cost-Effectivness for Healthcare
- P8910 - Introduction to Race and Health
- P8182 - Writing a Successful NIH Grant Application
- Once you have been registered for courses in late-June, you will be able to view your student account, view your invoice, and submit payments. The expected invoice date will be July 11 with a due date of August 7. Please note that billing is processed centrally and CSRI does not control invoices, payments, or late fees.
- CSRI participants are required to bring a laptop to classes. Our classrooms have wireless internet access. You may wish to prepare your laptop to connect to Mercury, Columbia University Irving Medical Center's secured wireless network. You may find more information and detailed instructions on setting up your wireless connection here.
- We will be using Stata 19, a versatile statistical analysis software program, in Biostatistics and Epidemiology labs, and TreeAge Healthcare Pro, a decision tree software program, in Decision Analysis. These software programs are compatible with both PCs and Macs. Please visit these pages to ensure your laptop meets the minimum system requirements for the software programs: Stata 19 and TreeAge. These programs will be purchased for you and you will receive software installation files and licenses in late-June after completing the CSRI 2025 Software Licenses Agreement by June 1.
- You will also need a basic scientific calculator. A fancy graphing calculator is not necessary; a calculator with logarithmic and exponential functions will suffice.
- Our classrooms are located in the Irving Institute Educational Center on the 10th floor of the Presbyterian Hospital Building (622 W 168th Street). Take the Presbyterian Elevators to the 10th Floor. When you exit the elevators, you will see double doors on one side of the elevator bank and a single doorway on the other side. Go through the single doorway. The educational center classrooms will be on your right and the administrative offices on your left. For a map of CUIMC, please click here.
