Timely, data-driven, high-quality, affordable care that improves health outcomes is often lacking in resource-constrained populations. Gaining the trust of communities requires consistent, connected care systems delivered on time to ensure survival for life, limb, and sight-threatening conditions. Countries navigating the triple burden of disease (e.g., injuries, communicable, and non-communicable diseases) require increased system inputs and expanded clinical acumen by frontline providers to ensure good patient outcomes. Currently, the systems to address this triple burden of disease are often uncoordinated, slow, and lack definitive care and effective referral systems. Inadequately integrated primary care, referral, and emergency care systems threaten the community's trust and willingness to seek care in the future. sidHARTe partners with governments, universities, and local organizations to ensure timely, integrated, adaptive health systems strengthening throughout an effectively linked continuum of care.
